Administrative Service Fees
Please be advised that in order for us to keep the cost of patient care as low as possible, our professional fees only relate to our professional services (physician and therapy). Administrative services are subject to a separate charge, generally require pre-payment, and may require a written medical release authorization. Such administrative services include, but are not limited to:
- Medical Record Copying
- X-Ray Copying
- Completion of Insurance Forms
- Completion of Health Assessment Forms
- Completion of Social Security Forms
- Attorney Information Requests
